What This Notice Means
By using iScript and providing us with your personal and health information, you are giving your informed consent to the collection, processing, and sharing of that information as described below — in accordance with the Protection of Personal Information Act 4 of 2013 (POPIA) and HPCSA guidelines.
Dr. Chomba & Associates, Inc (trading as iScript) operates as a group practice (BHF Practice Number 0558419) in South Africa. We provide telehealth consultation services through our platform connecting patients with qualified healthcare professionals. We act as the "Responsible Party" under POPIA for the personal information you provide to us.
When you use iScript, we collect: • Personal identification information (name, ID number, date of birth, contact details) • Health information including symptoms, medical history, medications, allergies, and chronic conditions • Consultation records including diagnoses, prescriptions, and clinical notes • Payment and medical aid information • Device and usage data Health information is classified as "Special Personal Information" under POPIA and is subject to enhanced protections.
By registering and using iScript, you consent to: ✅ Processing of your personal and health information to facilitate telehealth consultations ✅ Sharing your health information with the HPCSA-registered doctor assigned to your consultation ✅ Sharing your prescription information with a licensed partner pharmacy for medication fulfillment (where applicable) ✅ Submission of medical aid claims on your behalf (where you have provided medical aid details) ✅ Use of AI-assisted clinical assessment tools to analyse your symptoms and prepare a clinical report for your doctor ✅ Storage of your health records for the legally required retention period (minimum 6 years) ✅ Communication via email and SMS regarding your appointment, prescriptions, and health-related information
Your information is shared only on a need-to-know basis with: • Your consulting doctor (HPCSA-registered, bound by medical ethics and POPIA) • Partner pharmacies (for prescription fulfillment, with your consent) • Your medical aid scheme (for claims, where applicable) • Laboratory services (Ampath/Lancet) where investigations are ordered • Payment processors (PayFast) for secure transaction processing • Legal and regulatory authorities where required by law We do NOT sell, rent, or share your health information for advertising, marketing profiling, or any commercial purpose not directly related to your care.
As a data subject under POPIA, you have the right to: 🔍 Access: Request a copy of the personal information we hold about you. ✏️ Correction: Request correction of inaccurate or outdated information. 🚫 Objection: Object to processing of your information in certain circumstances. 🗑️ Deletion: Request deletion of your data (subject to mandatory health record retention periods). 📋 Portability: Request your health records in a portable format. ⚖️ Complaint: Lodge a complaint with the Information Regulator if you believe your rights have been violated. In accordance with HPCSA Booklet 10 (Guidelines for Good Practice in Telehealth), you acknowledge and consent to: • Receiving medical care via a telehealth platform • The inherent limitations of teleconsultation compared to in-person examination • The understanding that your doctor may refer you for in-person assessment if clinically required • That electronic records of your consultation will be maintained in accordance with HPCSA and legal requirements • That consultations may be recorded only with your explicit prior consent (obtained separately during booking)
If you are registering on behalf of a child under the age of 18, you confirm that you are the parent or legal guardian of that child. You provide consent on their behalf for the collection, processing, and use of their health information in accordance with this notice. POPIA requires specific care in the processing of information about children. We take this responsibility seriously and apply enhanced safeguards to all records of minor patients.
You may withdraw your consent to non-essential processing (e.g. marketing communications, Velobiotics recommendations) at any time by updating your preferences in the app or contacting practice@iscript.co.za. Withdrawal of consent to essential processing (e.g. sharing information with your treating doctor) will result in your inability to use the iScript consultation service, as it is impossible to provide safe medical care without this sharing.
